– Big Smile Dental Clinic Team
We’ve all heard the dreaded words: “The decay is too deep; you need a Root Canal and a Crown.” In many dental clinics, this is the standard “treatment of choice” once decay touches the pulp (the nerve of your tooth). It’s a reliable but extensive process that involves removing the nerve entirely, followed by crown, which makes it more expensive.

But what if there was a way to save the living heart of your tooth instead?
At Big Smile Dental Clinic & Prosthodontic Center in Kathmandu, Nepal, we believe in a different philosophy: Biomimetic and Regenerative Dentistry. We try to save tooth’s vitality whenever it is possible.
Recently, we managed a case where an X-ray (RVG) showed decay reaching right to the “pulp horn”—the very edge of the tooth’s internal tissue. but no infection seen in X-ray. While the traditional route would be to start a Root Canal immediately, we chose a more conservative and regenerative approach.
Using high-power magnification (Dental Microscope), we were able to see exactly where the unhealthy tissue ended and the healthy tissue began. During the cleaning process, the pulp was slightly exposed which was healthy. So, instead of removing the nerve, we performed Direct Pulp Capping assisted by Laser Therapy.

How Laser Technology Changes the Game
Think of the laser as a precision tool for biological healing. By applying specific laser wavelengths to the exposed area, we achieve three critical goals:
Decontamination: It kills bacteria that traditional tools might miss.
Biostimulation: It encourages the tooth’s own cells to repair themselves and form new dentin (the hard layer of the tooth).
Hemostasis: It controls bleeding instantly, allowing for a perfect seal.

By opting for this advanced, laser-assisted treatment over a traditional Root Canal, the benefits are clear:
Preserves Vitality: Your tooth remains “alive,” maintaining its natural sensation and strength.
Less Invasive: We avoid the extensive drilling and structural removal required for a crown.
Time Efficient: In this case, the entire tooth was restored with a final filling on the same day.
Cost-Effective: This approach is significantly less expensive than the combined cost of a Root Canal and a Crown.

The Bottom Line
Technology like high-magnification microscopy and lasers has shifted the boundaries of what we can save. A deep cavity is no longer an automatic death sentence for your tooth’s nerve. By choosing Regenerative Dentistry, we focus on healing the body from the inside out—saving your natural smile, one pulse of light at a time.
