Smile Transformation: Restoring Health and Esthetics 


A patient visited our clinic because she was unhappy with the appearance of her front teeth. Few years earlier, she had received a filling in her upper front tooth area but she was not satisfied with how it looked. The old filling had become noticeable and did not blend naturally with the surrounding tooth structure. During our clinical examination, we also identified dental caries affecting the adjacent teeth. Therefore, the treatment needed to address two concerns at the same time: eliminating the active decay and improving the appearance of the previously restored front tooth. The patient's main request was simple, she wanted her front teeth to look healthier, more natural and harmonious when she smiled.

Initial Examination

A detailed clinical examination of the anterior teeth was performed before beginning treatment. The existing restoration was evaluated for its margins, color, contour, and relationship with the surrounding natural tooth structure. The adjacent tooth was also examined, where a carious lesion was identified. Because the front teeth play such an important role in the smile, our treatment planning focused not only on removing disease but also on achieving a natural result in terms of tooth shape, tooth proportion, shade, surface texture, symmetry, contact between the teeth.

Treatment Planning

After discussing the findings and treatment possibilities with the patient, a conservative esthetic treatment plan was developed. The objectives were to:
- Remove the old unsatisfactory restoration and carious tooth structure
- Preserve as much healthy natural tooth structure as possible
- Restore the affected areas
- Improve the shape, color, and overall appearance of the front teeth
- Place an esthetic veneer where indicated
- Finish and polish the restorations so that they blended naturally with the patient's smile

The patient understood the proposed treatment and agreed to proceed.

Shade and Esthetic assessment

Before beginning the restorative procedure, the color and overall appearance of the surrounding natural teeth were evaluated.
Shade selection is especially important in the anterior region because even small differences in brightness, translucency, or color can make a restoration noticeable.
The neighboring teeth were used as a reference so that the new restoration and veneer could be designed to integrate naturally.
We also evaluated the length, width, contour and symmetry of the front teeth before treatment.

Removal of the old filling and carious tooth structure

The previous filling was carefully removed.
Once removed, we were able to evaluate the remaining healthy tooth structure and establish a clean foundation for the new restoration.
Special care was taken to preserve healthy enamel and dentin wherever possible.
The decayed and compromised tooth structure was carefully removed while maintaining as much healthy natural tooth structure as possible.
Once the caries had been completely removed, the remaining tooth structure was restored with tooth colored filling, composite.

Digital Veneer Design Using exocad

The intraoral scan data were imported into exocad Dental CAD for digital veneer design. The veneer was adjusted to achieve appropriate tooth shape, symmetry, incisal length, proximal contacts, and facial contour. Veneer thickness and emergence profile were evaluated to avoid overcontouring, while occlusal contacts and interferences were checked and refined. After final assessment of esthetics, marginal adaptation, and occlusion, the completed design was exported for CAD/CAM fabrication.

Veneer Placement

To further improve the appearance of the front tooth, esthetic veneer was placed. The veneer allowed us to improve the visible facial surface of the tooth and create better harmony in its color, contour, and proportion. Before final placement, the veneer was evaluated in relation to the neighboring teeth and the patient's overall smile. Once the desired position and appearance were confirmed, the veneer was bonded according to the appropriate adhesive protocol. Any excess material around the margins was carefully removed.

Final Evaluation





Once treatment was complete, the final restorations were evaluated from different angles. We assessed overall tooth shape and proportion, shade, symmetry, contact areas, marginal finishing, surface texture, occlusion and overall smile harmony. The previously noticeable filling had been replaced, the decay in the adjacent tooth had been treated, and the veneer provided a more harmonious appearance to the anterior region.

Before and After

The transformation was especially noticeable when comparing the initial and final photographs.

Before: The patient presented with an older anterior restoration that did not provide the appearance she desired, along with caries affecting an adjacent front tooth.

After: Following removal of the previous restoration and dental caries, the teeth were restored and an esthetic veneer was placed to improve the color, contour, and overall harmony of the smile. The final restorations blended much more naturally with the surrounding teeth while maintaining a conservative approach to the patient's natural tooth structure.

The Final Outcome

The primary goal of this case was to combine dental health with natural looking esthetics. Rather than focusing only on replacing the old filling, the treatment addressed the entire clinical situation; the defective previous restoration, the newly identified caries, the anatomy of the affected teeth, and their relationship with the patient's smile. At the completion of treatment, the patient was very pleased with the improvement. The new restorations appeared more harmonious with the surrounding natural teeth, and the veneer helped create a cleaner and more balanced appearance of the front teeth. Most importantly, the caries was treated and the compromised restorations were replaced while preserving healthy tooth structure wherever possible. The patient left our clinic happy and satisfied with her new smile.
