Restoring a Failing Smile: A Full Mouth Implant Journey in Kathmandu

Recently, a middle-aged Nepalese gentleman living overseas returned to Kathmandu seeking a definitive solution for his teeth. For over two years, he struggled with severe tooth mobility across both jaws, which heavily impacted his ability to chew comfortably and smile confidently.

Assessment & Advanced Digital Planning

Our clinical and 3D CBCT imaging revealed severe bone loss due to long-standing infection, particularly affecting the lower jaw and the upper right side. Fortunately, his upper left teeth were perfectly stable and healthy.

Instead of an aggressive “pull everything” approach, we designed a highly customized, conservative treatment plan:

  • Upper Arch Restoration: Due to localized bone loss from infection, conventional implants lacked support on the right. We planned 1 Zygomatic Implant (anchored securely in the cheekbone) combined with 2 Conventional Implants on the front right, plus a single implant to replace missing tooth #26. The healthy upper left teeth were completely preserved.

  • Lower Arch Rehabilitation: A comprehensive All-on-6 implant protocol was selected to entirely rebuild his failing lower dentition.

Smooth Surgery & Immediate Fixed Teeth

Because the entire procedure was digitally mapped out beforehand, the surgery was smooth, efficient, and completely comfortable for the patient.

In a single visit, we removed the failing, loose teeth and placed the implants. The very next day, we fitted his customized temporary teeth: an implant-supported fixed partial bridge for the upper right, and a fixed All-on-6 provisional denture for the lower arch. The patient was thrilled to have his smile and chewing function restored instantly before traveling back.

The Road to a Permanent Smile

The patient is currently practicing strict oral hygiene maintenance as advised to safeguard the healing sites. After the integration period is complete and the bone successfully fuses with the implants, he will return for a follow-up recall visit. At that stage, we will seamlessly exchange his temporary provisional teeth for his final, highly durable permanent prostheses.
